All Singapore travelers and visa applicants are required to apply for the Arrival card. We suggest you to submit this form when you apply for your Singapore visa with us, so we can process this application on time. Please note that the form should be submitted three days before your arrival date.

Penalties for failure to submit or incorrect information by the ICA Failure to complete/submit this card:
- Fine of up to S$10,000 and/or imprisonment of up to 6 months
- Fine of up to S$20,000 and/or imprisonment of up to 12 months (subsequent offense)
Providing false information:

Note for overseas digital vaccination certificate:
If you have an overseas digital vaccination certificate, you should upload/scan the QR code for your final dose from your vaccination certificate (NOT the scanned copy of the certificate), for your vaccination status to be reflected in your Health Hub and/or Trace Together app after immigration clearance.

If you do not have or are unable to upload/scan the digital vaccination certificate, please present your physical/digital vaccination certificate for checks by the authorities upon arrival in Singapore.

You do NOT need to upload your digital vaccination certificate if you are vaccinated in Singapore.
